  Das mit den Erythrozyten entfernte Eisen führt zu einem sekundären Eisenmangel, der die Erythropoese limitiert und zu Müdigkeit und Abnahme der Leistungsfähigkeit führen kann.
  
  Zunächst im Osten Russlands und später auch in Mitteleuropa fand man eine besondere Kind der erblichen Polyzythämie.
  Ihr liegt eine autosomal-rezessiv vererbte Mutation im Von-Hippel-Lindau(VHL)-Gen zugrunde.
  Das VHL-Protein reguliert in den peritubulären Fibroblasten der Niere
  den Abbau des Hypoxie-induzierbaren-Faktors (HIF1) (Grafik 2).
  Dieser besteht aus 2 Untereinheiten a und b und vermittelt die Sauerstoffmessung in den Nieren. In Gegenwart von Sauerstoff werden die a-Ketten unter Beteiligung des VHL-Genproduktes abgebaut
  (3).
  NGS ist eine molekulardiagnostische Methode, die erlaubt,
  mehrere Gene gleichzeitig zu untersuchen und
  auch vor einer KMP durchgeführt werden kann. Angeborene sekundäre Erythrozytosen sind selten und
  werden meist durch Keimbahnmutationen verursacht, einschliesslich Mutationen in Genen, die am Sauerstoff-Sensorweg und Hb-Bildung beteiligt sind, unter anderem abnormale Sauerstoffaffinität (4).
  Es sind mehrere Mutationen bekannt, welche eine kongenitale Erythrozytose verursachen können (Tab. 4).
  Die sekundäre und die idiopathische Polyzythämie benötigen aus hämatologischer Sicht keine Therapie, da diese Patienten prinzipiell kein klar erhöhtes Risiko für Thromboembolien oder Transformation in eine andere
  Erkrankung haben. Eine Knochenmarkpunktion (KMP) ist hilfreich,
  um den Typ der MPN zu charakterisieren und das Vorliegen einer Fibrose,
  die prognostische Relevanz hat, zu prüfen. Wichtige
  Aspekte bei der Beurteilung sind die gesteigerte Zellularität, eine trilinäre
  Myeloproliferation, die Morphologie der Megakaryopoese, das Vorhandensein einer Fibrose, sowie
  eventuelle Vermehrung der Blasten. Die Flowzytometrie ist nicht zwingend nötig, könnte aber dazu beitragen,
  eine Zunahme der Blasten auszuschliessen.
  Steht die alleinige Senkung der Thrombozyten zur Diskussion, ist
  Anagrelid das Medikament der Wahl (mittlere Dosierung 2 mg täglich).
  Dieses Präparat wirkt selektiv auf Megakaryozyten und ist nicht leukämogen. Nebenwirkungen können Kopfschmerzen und Palpitationen sein (23), die im Allgemeinen nach four Wochen wieder abklingen. Aufgrund der oralen Verfügbarkeit, der
  besseren Verträglichkeit, des geringen Preises und der Zulassung wird im Allgemeinen die Gabe von Hydroxyharnstoff
  gegenüber der von Interferon-a bevorzugt. Konventionelles Interferon-a und pegyliertes Interferon-a (kovalent an einen Polyethylenrest gekoppeltes Interferon-a) verbinden hohe Wirksamkeit mit fehlendem leukämogenen und tumorogenen Risiko.
  IE Interferon-a subkutan (beziehungsweise forty µg pegyliertes Interferon-a) pro Woche
  reduziert es Erythrozytose, Splenomegalie und Pruritus.
  
  Labordiagnostisch zeigt sich das Bild einer Polyglobulie, das jedoch
  nur vorgetäuscht ist, da in Wirklichkeit ein Flüssigkeitsmangel und damit eine Dehydratation vorliegt.
  
  Eine Polyglobulie ist eine Erhöhung der Zahl der roten Blutkörperchen (Erythrozyten) über
  den physiologischen Normwert. Sie macht sich labordiagnostisch durch
  einen erhöhten Hämatokrit bemerkbar. Prof. Dr. Petrides hat Vortragshonorare von den Firmen Shire und AOP erhalten. Herr
  Siegel erklärt, dass kein Interessenkonflikt im Sinne des International Committee
  of Medical Journal Editors besteht. Der wasserinduzierte Juckreiz ist das chronische Symptom der PV, welches die Lebensqualität am stärksten beeinträchtigt.
  
  DHT entsteht beim Abbau von Testosteron und ist das
  biologisch wirksamere Hormon. Es kann zu dem sogenannten erblich bedingten Haar­ausfall (der androgenetischen Alopezie) führen, worunter etwa die Hälfte aller Männer
  ab einem bestimmten Alter in unterschiedlicher Ausprägung leidet.
  Grund hierfür ist die erhöhte Empfindlichkeit der Haarwurzeln gegenüber Dihydrotes­tosteron, die eine
  verstärkte Schädigung und Schwächung der Haarfollikel bedingt.
  
  Unser vorgeschlagener Diagnosealgorithmus für JAK2-unmutierte
  Polyzythämie bei Erwachsenen ist in Abbildung 1 dargestellt.
  Zusammenfassend kann gesagt werden, dass die Poly­zythämie und Erythrozytose wichtige Laborbefunde sind, welche eine weitere Abklärung triggern sollten. Parameter, die für die
  Kategorisierung dieser Artwork von Polyzythämie wegweisend sind, umfassen den Epo-Spiegel im Serum
  und den P50-Wert.
  Die JAK2 (Januskinase 2) ist eine zytoplasmatische
  Tyrosinkinase, die an der Signaltransduktion verschiedener Zytokine (unter anderem am
  EPO-Rezeptor) beteiligt ist. Die Mutation verstärkt die
  Aktivität der JAK2 und führt damit zu EPO-unabhängigem
  Wachstum (Grafik 3). Diese Mutation ist erworben, weil
  sie nicht in der Keimbahn nachweisbar ist.
  Da Erythrozyten keinen Zellkern mehr besitzen, erfolgt der Nachweis dieser JAK2V617F-Mutation im peripheren Blut über
  Granulozyten-DNA. Verschiedene Nierenfunktionsstörungen können auch ohne hypoxische Stimuli zu
  einer Erythrozytose führen.
  Bestimmte Mutationen im Bereich der a- beziehungsweise b-Ketten des Hämoglobins
  können zu Hochaffinitäts-Hämoglobinen führen, die Sauerstoff im peripheren Gewebe in verringertem Maße freisetzen. So liegt bei einem O2-Partialdruck von 20 mm Hg im
  Kapillarbereich 35 % Oxyhämoglobin vor, wohingegen zum Beispiel beim Hochaffinitäts-Hämoglobin Johnstown noch 60 % oxygeniert sind
  (Grafik 1). Die dadurch verursachte Verringerung der Sauerstoffabgabe im peripheren Gewebe führt zu einer
  kompensatorischen Erhöhung der Hämoglobin-Konzentration. Daneben kann auch eine Erniedrigung des intraerythrozytären 2,3-Bisphosphoglyceratspiegels durch 2,3-Bisposphoglyceratmutase-Mangel
  zu einer erhöhten Sauerstoffaffinität des Hämoglobins führen.

  Giselle Ruemke was a Canadian traveler in her 50s who had, it turned out, a number of things in common with Savery Moore.

  In the wake of her grief, booking the trip on The Canadian seemed, to Giselle, “like a good way to connect with myself and see my country, refresh my spirit, a little bit.”
  
  Like Savery, Giselle had always dreamed of taking the VIA Rail Canadian with her late spouse. And like Savery, she’d decided traveling solo was a way of honoring her partner.
  
  “That trip is something that I would have really liked to have done with my husband, Dave. So that was why I was taking the train,” Giselle says today.
  
  But unlike Savery, Giselle hadn’t booked prestige class. She admits she was “sticking it to the man” in her own small way by sitting in the reserved seats that first day.
  
  She’d only moved when Savery arrived. She tells CNN Travel, laughing, that she’d thought to herself: “I better get out of the seat, in case someone prestige wants to sit in that spot.”
  
  Giselle didn’t tell Savery any of this in their first conversation. In fact, she didn’t share much about her life at all in that first encounter.
  
  But Giselle liked his company right away. He was friendly, enthusiastic and respectful — sharing that he was a widower and indicating he knew about Giselle’s loss without prying about the circumstances.
  
  As for Savery, he says, it was “the common bond, the losses of our respective loved ones” that first made him feel a connection to Giselle. But it was also obvious that for Giselle, the loss was much fresher. She clearly didn’t want to talk about Dave that day.

  The reservedness that marked their first meal together had all but melted away. It was an evening marked by laughs, recalling favorite memories of the trip across Canada and talking about their lives back home.
  
  The next day, they said goodbye. Appropriately enough, their farewell took place at a train station.
  
  “I was taking the airport shuttle to fly back home to Boston, and Giselle was taking the train to Montreal. So we said, ‘Well, let’s just say goodbye at the train station, since we’re both going to be there at the same time tomorrow,’” recalls Savery.
  
  “We were under the big clock in Toronto station, and she was watching the clock. She said, ‘I really gotta go. I have to catch my train.’ And I just… I said, ‘I can’t not see you again.’”
  
  Their connection didn’t feel romantic — both Giselle and Savery were sure of that. But it felt significant. Both Savery and Giselle felt they’d met a like-minded soul, someone who could be a confidant, who could help them through the next chapter of life which they were unexpectedly navigating alone.
  
  Saying “goodbye” felt too final. So Giselle, who is French-Canadian, suggested they say “au revoir” — which translates as “until we meet again.”
  
  And as soon as they went their separate ways, Giselle and Savery started texting each other.
  
  “Then the texts became phone calls,” recalls Savery.
  
  On these calls, Giselle and Savery spoke about their lives, about what they were up to, about their interests.
  
  “Music was like a common interest that we both shared,” recalls Giselle.
  
  Savery is older than Giselle, and their music references spanned “different eras of music, but very compatible musical interests,” as Giselle puts it.
  
  On one of their phone calls, Giselle mentioned she was considering booking a train trip across North America.
  
  Soon, she and Savery were planning a train journey across the US for the fall of 2024, together.
  
  And in the meantime, Giselle invited Savery to visit her in her home in Victoria, Canada, for a week’s summer vacation.
